Disclaimer, Waiver I, the undersigned hereby waive all claims against any and all persons associated with learning self-defense, fitness and Jiu-Jitsu martials arts with the coach and supporting trainers. - I understand that I am participating in a martial art that has body contact. - I understand that this form of martial arts involves but is not limited to; arm locks, throws, grappling and such which could potentially cause serious injury or even death. - I assume full responsibility for any children within my responsibility, for actions during and connected to the classes above. - I understand the risk of participating in this form of martial arts training and hereby release and hold harmless the above coaches or trainers, volunteers, members, guests, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of real property and personal property used to conduct the sessions/classes, from any and all liability, claims, demands, actions, medical bills, and causes of action whatsoever arising out of or relating to any negligent or other act of omission, and or any loss, damage, or injury, including death, that may be sustained by the undersigned or any property of the undersigned in participating in the sessions/classes. - I the undersigned also state that my child/ or child I am enrolling is in good physical condition and know of no reason why he/she cannot train in this form of training. - I understand that in case of emergency, I hereby authorize any licensed medical personnel to perform any accepted medical assistance deemed necessary and I agree to bear the expense of any such treatment. - I acknowledge that the instructors/coaches above are not medical experts. By accepting this waiver, and to the best of my knowledge, I confirm that I, and/or the children in my care who are participating do not have any medical condition that could make sustaining an injury more likely. - As additional consideration for training at the above classes, I agree that my attendance and or performance at the classes and abroad at events and such may be photographed, filmed and or taped and used for marketing purposes and I authorize the use of my image and I waive any compensation thereof – even if my child discontinue their training above. - I, the undersigned, being duly aware of the risks and hazards inherent upon participating in this form of martial arts, self defense and fitness sessions, agree

I, the undersigned parent or legal guardian, understand that participation in the Kids' Jiu-Jitsu Self-Defense Class involves physical activity and carries certain inherent risks. I give permission for my child to participate and agree to the following: 1. Assumption of Risk I understand that Jiu-Jitsu includes physical contact and techniques such as falls, grappling, and submissions, which may result in injury. I acknowledge that my child is participating voluntarily and that I accept full responsibility for any injury or harm that may occur. 2. Release of Liability I release and hold harmless the instructors, assistants, volunteers, participants, and affiliates from any liability or claims arising out of my child’s participation. I understand that the Al Fatiha Center is not responsible for any injury, accident, or harm that may occur during or as a result of this program. 3. Medical Consent I confirm that my child is physically fit and has no medical conditions that would prevent safe participation. In case of emergency, I authorize the instructor or any adult present to obtain medical care for my child and agree to bear any related costs. 4. Illness & Communicable Disease I understand that my child may be exposed to illnesses including but not limited to COVID-19, flu, or skin infections. I agree not to bring my child to class if they are sick or have been exposed to a communicable illness. I will not hold the instructors, class organizers, or the Multicultural Center responsible for any illness contracted during participation. 5. Agreement I confirm that I have read and understood this waiver and that I am signing voluntarily. I agree to all terms listed above.
